You may be eligible for same-day London collection if you are currently in London, up to 9 weeks and 6 days pregnant on the day of treatment, and a doctor confirms that medical abortion at home is clinically appropriate for you. Our London collection pathway is designed for patients who:
- Are currently in London or able to collect from South London: Your medication is collected from our partner pharmacy in Balham after your consultation is approved.
- Can complete their consultation before the collection cut-off: For same-day collection, your consultation must usually be completed and approved before 3:00 PM Monday–Friday.
- Want private care without a GP referral: You do not need a GP referral, NHS number, permanent UK address or current GP registration to access our private service.
- Are suitable for medical abortion at home: Your doctor will review your pregnancy dates, medical history, current medication, allergies and symptoms before approving treatment. If our doctor decides treatment is not clinically appropriate, we will explain the safest next step and process your refund in line with our refund policy.
A scan is not routinely required if your dates are clear and there are no clinical concerns. Your doctor will calculate your pregnancy timeline from the first day of your last menstrual period and assess whether treatment can safely proceed.
If there is uncertainty about your dates, symptoms that need checking, or a possible risk of ectopic pregnancy, your doctor may advise a scan before treatment. If that happens, our team will help coordinate it before your medication is released.
Before approving abortion medication, your doctor checks that early medical abortion at home is safe and clinically appropriate for you. This assessment is completed during your confidential phone or video consultation and focuses on the key safety checks required before prescribing mifepristone and misoprostol.
Pregnancy dates: Your doctor confirms how many weeks pregnant you are, usually from the first day of your last menstrual period.
Symptoms: Your doctor will ask about pain, bleeding, shoulder-tip pain, dizziness, fainting or other symptoms that may need further assessment.
Medical history: Your doctor reviews any relevant health conditions that could affect whether medical abortion is suitable for you.
Current medication and allergies: Your doctor checks whether you take any medication or have any allergies that may affect treatment.
Contraindications: Your doctor screens for reasons why mifepristone or misoprostol may not be clinically appropriate.
Consent and understanding: Your doctor confirms that you understand how to take the medication, what to expect, and when to seek help.
Routine blood tests or STI tests are not usually required before early medical abortion at home in England and Wales. In line with RCOG best-practice guidance, extra checks are only advised when your pregnancy dates, symptoms, medical history or clinical risk factors suggest they are needed.
Medical abortion is highly effective, but in a small number of cases, further medication, a scan or additional treatment may be needed.
A second dose may be recommended: Your treatment pack includes 2 extra misoprostol tablets. Your doctor may advise you to take these if clinically appropriate and based on your symptoms and progress. Using an additional dose of misoprostol when needed is consistent with RCOG best practice guidance for medical abortion care.
A scan may be needed: If bleeding does not start as expected, symptoms continue, or your doctor needs to confirm what has happened, we may arrange a scan before advising the next step.
Further treatment is uncommon: Top at Home’s published 2025 outcomes reported a 99.35% overall treatment success rate and a 0.22% surgical intervention rate.
You can contact us at any stage: If you are unsure whether the treatment has worked, symptoms feel unusual, or you are worried, contact our clinical team directly for advice.
