What is Misoprostol? A Simple, Reassuring Guide
If you’re considering a medication abortion, you’ve likely heard of Misoprostol, the second pill taken after Mifepristone. But what exactly does Misoprostol do, how does it work, and what can you expect? At Top at Home, we’re here to provide medically-accurate, clear, and compassionate guidance, so you can make informed choices that are right for your body.
Medication abortion is legal and accessible in the UK up to 10 weeks of pregnancy. Misoprostol is a medication used alongside Mifepristone to safely end an early pregnancy. After taking Mifepristone, Misoprostol is usually taken within 24 to 48 hours. It works by causing the uterus to contract, helping the body pass the pregnancy tissue in a process similar to a natural miscarriage. Misoprostol is trusted worldwide and is recommended by leading medical authorities such as the World Health Organization (WHO) and the Royal College of Obstetricians & Gynaecologists (RCOG).
Used in over 70 countries, Misoprostol is an essential part of safe abortion care. When used with Mifepristone, it is approximately 98% effective. In the UK, it is approved for safe, at-home use under medical supervision.

What is Mifepristone? A Simple, Reassuring Guide
How Does Misoprostol Work?
A medication abortion typically involves two medications. First, Mifepristone is taken to block the hormone progesterone, which is necessary to maintain a pregnancy. Without this hormone, the pregnancy cannot continue. Around 24 to 48 hours later, Misoprostol is taken to complete the process. Misoprostol stimulates the uterus to contract, leading to cramping and bleeding that expels the pregnancy tissue.
As Dr. Albert Aka, Consultant Gynaecologist at Top at Home, explains, "Misoprostol plays an essential role in completing the abortion process. It helps the body release the pregnancy safely and effectively in a way that mimics a natural miscarriage."
How to Take Misoprostol
At Top at Home, we provide personalised instructions to ensure safe and effective treatment. Typically, you will be advised to take four tablets of Misoprostol (800 micrograms). Two tablets are placed on each side of your mouth between your cheek and gum. Allow them to dissolve slowly for about 30 minutes without swallowing. Afterward, you can swallow any remaining fragments with water.
After taking Misoprostol, cramping and bleeding usually begin within 1 to 4 hours. It’s normal to experience heavy bleeding, including the passing of clots, as the uterus contracts. Cramping can be intense but is often manageable with ibuprofen or other pain relief options. Some people also experience nausea, diarrhoea, chills, or a mild fever.

What to Expect After Taking Misoprostol
Everyone’s experience with Misoprostol is slightly different. Most people will experience strong cramping and heavy bleeding for a few hours, often passing clots or tissue during this time. These are signs that the medication is working as intended.
Many people complete the abortion within 4 to 6 hours, though it can take longer for some. Bleeding may continue for up to two weeks, gradually becoming lighter over time. Feeling tired or emotional is also common, and it’s important to rest and listen to your body during this process.
Dr. Aka advises, "Use this time to rest, hydrate, and listen to your body. Keep a hot water bottle nearby, and don't hesitate to reach out for support if you need it."
What are the side effects of Misoprostol?
When to Contact a Doctor
While side effects like cramping, bleeding, and mild fever are expected, you should seek medical help if you experience heavy bleeding that soaks through two or more pads per hour for two or more hours, severe abdominal pain that doesn’t improve with painkillers, a fever over 38°C lasting longer than 24 hours, or foul-smelling vaginal discharge.
At Top at Home, we offer 24/7 aftercare and virtual support so that you have expert help whenever you need it.
How Effective is Misoprostol?
When used with Mifepristone, Misoprostol is approximately 98% effective at completing a pregnancy termination without the need for surgery. In rare cases where the abortion is incomplete, a second dose of Misoprostol or a follow-up procedure may be necessary.
You can confirm that the abortion was successful by taking a pregnancy test 3 to 4 weeks after treatment. It’s important not to test earlier, as pregnancy hormones can remain in the body for a few weeks, leading to false positives. Most people find that their period returns within 4 to 6 weeks, a natural sign that the body is returning to its regular cycle.
